How to Improve my APFT and Run Time without Killing my Knees and Back - A Soldier's Story

As an Army Soldier Medic (25 yrs) I ran many miles with and without a ruck. I was on jump status for 6 years and I've attended some of the Army's toughest schools. I even broke my foot in one of those schools during the final 21 mile ruck march. But I did not quit. I finished that march and the course. That's hard as woodpecker lips. Well, some might thing so. I just wanted to finish. 
I inherently have bad knees, back, shoulders. 
Yet, at the age of 42 and 23 years into my 25 year enlisted time, I was able to use this program to shave 2 full minutes off my two mile run, max my sit-ups and push-ups. And I did this in just three week without running and zero sit-ups for the 6 months prior. 
As much as would like to say Insanity is for everyone, you know as well as I do, that's simply not the case.
